Overview

During this presentation, Laura Nelson, PharmD, discusses length of therapy (LOT) and why “shorter is better” is the new mantra.  She reviews beta-lactam de-escalation and how it impacts new gram-negative resistance in septic adults.  Dr. Nelson discussed studies and articles related to AKI risks with Vancomycin used in conjunction with other beta-lactams in critically ill patients.  She reviewed antibiotic dosing in patients with severe sepsis or septic shock.  She drew attention to a new best practice alert in EPIC flagging physicians when they order Vancomycin or linezolid for patients with pneumonia.  Lastly, she drew attention to the need for blood culture stewardship in light of the culture supply shortage. 


Objectives
At the conclusion of this activity, learners will be able to:

  1. Understand and illustrate the new antibiotic mantra “shorter is better.”
  2. Describe and illustrate research related to patient outcomes for shortened LOT
  3. Explain and illustrate the current vancomycin stewardship program
  4. Explain and illustrate the current blood culture stewardship procedures
