Weekly review of new patients and physician discussion on which pathway they would be best served, medical management; MCS of transplant

1. Utilize and enhance the knowledge base of physicians and allied health professional in order to achieve the best possible outcome for heart failure patients with end stage disease.
2. Understand the various aspects to consider when developing treatment plans for difficult cases.
